I downloaded and approved an update with SUS. I’ve just realised that I approved the wrong update so I’ve unapproved it but the update hasn’t been de-installed from those clients that have already installed it?

This is normal behaviour. SUS doesn’t remove/ uninstall any updates that have been previously approved.
