Microsoft uses a geographically distributed caching network to host the downloads near your physical location. This improves the download speed significantly.

If you have multiple sites in different locations in the world and all your clients have access to internet then it is best to keep the packages on Microsoft servers. This way each of the Auto Update clients downloads the package from a location near them.

You can approve or unapproved packages just like if they were downloaded to your machine.
